 Community Service Requests

    As a community service volunteer, you are required to notify your site supervisor of the following information on the first day that you volunteer by submitting this form to them.  If you do not submit this form, we can not sign or provide community service hours acknowledgement. Please note on the form who the Site Supervisor was. In addition, we ask that you fill out the volunteer waiver form prior to your community service so we have time to conduct a sex offender and criminal background check.

    To receive a formal letter of acknowledgement for the hours contributed, contact Debra Stacey at deb@habitatriwestbay.org or call 401-732-6407.  Your letter request must be made after your hours are completed.   Please allow one week for the letter to be created. 

    You must be 16 years of age to volunteer. If you are 16, you need to be accompanied by an adult. You may skip to the bottom for a required signature. Make sure you know who your site supervisor is/was
    If this is for a court order, please fill out the remaining questions
    Habitat for Humanity of West Bay and Northern RI is happy to have many different roles in volunteering. Some volunteering roles may be limited based on the crime you were convicted of.  No court ordered volunteers are permitted to handle money or do pickups of donated/purchased items.
